I/We hereby release, indemnify and hold harmless the Mesa Soccer Association Nott's Forest F.C./League and its Boards of Directors, Members, Officials, Tournament Sponsors, Coaches, Referees and all other representatives of this organization from any claim arising out of this competition including tournament scheduling, monetary, physical or emotional injury to the team participants. Your application to participate will not be considered without a fully completed form and payment, $550 for U9 - U10 and $575. for U11-U19. The deadline for guaranteed entry is July 25, 2008. Beyond that date, entries will be accepted, but only to fill open brackets. Check our brackets page for bracket openings. We recognize that in case of adverse weather, all decisions regarding playability of facilities by the Tournament Director are final without objection, appeal or compensation.
