Roster Game Card Instructions

When the Roster Game Card is open, please click File, then "Save As" a "My Document".  Name the file as an Example:  GU12 Gold Notts Forest Red.  Complete the form as instructed below and then email as an attachment to laborday@san.rr.com no later than Aug 20th. The form cannot be emailed from the website but must be saved as a "my document" and then emailed as an attachment. 

All the information can be found on your team schedule:

  1. TEAM NAME - ALL CAPS
  2. AGE/BRACKET you will be playing in as indicated on your schedule.
  3. Coaches Name
  4. Managers Name
  5. Contact phone number while at the tournament. 
  6. Jersey # and players name - Last name first, alphabetical order
  7. Most teams will be playing 3 games. If you are in a 5 team bracket, you will be playing 4 pool games.
  8. Some teams will play 2 games on Saturday, 1 on Sunday. others will play 1 game on Saturday, 2 on Sunday. Please include field name AND field #. (i.e. Robb #6)
  9. All semi's and finals are on Monday. List teams as listed on the schedules (i.e. WINNER SEMI 1, WINNER SEMI 2 etc), times, fields in semi section. If you do not have a semi-final, leave that section BLANK.
  10. Home team is listed 1st on the schedule and should be listed 1st on the game card. If you are listed 2nd on the schedule, you should be listed 2nd on the game card.
  11. Please do not change the format of the card or the font size.

You will sign your Roster/Game card at registration when all the information is verified.
